What is this survey about?

This is a very important survey conducted every three years by Weld County Department of Public Health and Environment. It helps the Health Department and its partners learn about the health needs in our county and your neighborhood. Your participation can help improve the health of county residents.

How are we going to use this information?

Results from the survey will be used to help local health and human service organizations do a better job providing services and improving the well-being of residents by understanding the health needs of residents. Previous survey results have been used to make Weld County a healthier place to live, learn, work, and play and can be found online at www.weldhealth.org.

What specifically will we ask?

The survey asks about physical and mental health conditions, habits, and behaviors, health insurance including barriers to accessing health care, opinions about environmental health issues, housing and needs related to health and well-being.

How was I selected to be in the survey?

Using a scientific random sampling method, we selected your household from one of a few in your community to provide information about health-related issues in Weld County.

Who sees my answers?

Your answers are confidential. You are not asked to provide your name or other identifying information. Only authorized employees of the Health Communication, Education, and Planning Division at the Health Department will have access to survey results. Your answers will be grouped with others' and cannot be traced back to you as an individual.

How much time will the survey take?

Based on preliminary tests, for most people, the survey takes about 10-20 minutes to complete. Since not everyone will complete all parts of the survey, actual times do vary.

What happens if I don't answer?

This is a voluntary survey. However, if you do not answer, it could weaken the results of the survey. Your answers will represent many other residents in Weld County. We dedicated substantial resources to ensure as complete a response as possible, including multiple mailings. We hope you will respond.

Can I see a report from the survey?

Can I get an reward for completing the survey?

By completing the survey, you can receive a $5 reward card as a thank you for your participation. At least 50% of the survey questions need to be answered to receive the reward. Several Weld County healthcare partners including Banner Health, Intermountain Health, North Range Behavioral Health, Sunrise Community Health, and UCHealth provided funds for the reward program.

See the last page of the online survey for more details about claiming your digital reward. If you complete and return a paper version of the survey you will receive your reward card in the mail within 3 weeks.

Are Weld County tax dollars paying for the survey reward?

No, funding for the entire cost of the survey reward program has been provided by several Weld County healthcare partners. Banner Health, Intermountain Health, North Range Behavioral Health, Sunrise Community Health, and UCHealth provided monetary donations to the Health Department for the reward program because they value the Community Health Survey and believe it helps improve the health of county residents.

I received a Colorado Health Access Survey AND a Weld Community Health Survey.
Are these the same survey?

No, these are separate surveys. A statewide health survey, called the Colorado Health Access Survey, is being conducted at the same time as the Weld Community Health Survey. A small number of households in Weld County may receive both surveys. While they may seem similar, the Weld Community Health Survey provides our county with much richer and more detailed information on the health of our residents. While both of these surveys are important, it is up to you whether you fill out both, just one, or neither.
